Ask for a rebate claim form at the sales counter of the retail store where you have purchased your Envision display.

Read the rebate claim form very carefully and keep your purchase receipt and the box in which your display was packaged.

Complete the rebate claim form with the required information. Gather your completed claim form, purchase receipt (or online purchase confirmation), and original UPC bar code (cut from the box). Make a copy of all your paperwork before enclosing the documents in one (1) envelope.

Mail your completed rebate claim form, purchase receipt, and original UPC bar code to the address (with department number) indicated on your rebate claim form. Your rebate claim submission must be mailed BEFORE the indicated postmark date. NOTE: Claims received by our Fremont, CA facility will not be processed.
Allow 10-12 weeks for rebate processing (not including mail time).
